Accountant for nurses

Accountant for Nurses and Healthcare Workers

We prepare returns for staff and per diem nurses, travel nurses, medical residents and home health aides who live or work in New York City. Healthcare returns get complicated fast: several W-2s, more than one state, stipends and a side gig that nobody withholds tax on.

Several W-2s and per diem shifts

Working per diem at two or three hospitals or agencies means each employer withholds as if it were your only job. The result is often too little withholding and a balance due in April. We check your pay stubs during the year and tell you whether to adjust your W-4 or IT-2104. If you earn enough across employers, too much Social Security can be withheld; the excess over the 2026 wage base of $184,500 comes back as a credit on your return.

The federal overtime deduction

For 2026 you may be able to deduct up to $12,500 of qualified overtime ($25,000 on a joint return), reduced once modified AGI is over $150,000 ($300,000 joint). It covers only the extra half of overtime pay required by federal wage law, not your whole overtime check. Employers report it on your W-2 in box 12 with code TT. If you are married, you must file jointly to claim it.

Travel nurses: stipends and tax home

Housing and meal stipends stay tax-free only if you have a tax home you keep paying for while you travel. Without one, the stipends are taxable wages. You also usually file in more than one state: a nonresident return where you worked and a resident return at home. If New York is the work state, see our page on New York nonresident returns.

Union dues and work expenses

Employees cannot deduct union dues, scrubs or license fees on the federal return; that deduction is gone. New York is different. If you itemize on your New York return, which you can do even if you take the standard deduction federally, union dues and other unreimbursed job expenses can count, subject to the same 2% of income floor that applies to other job expenses. We check whether itemizing for New York saves you money.

Common questions

I'm a travel nurse working in NYC. Do I file a New York tax return?

Usually yes, as a nonresident on Form IT-203, paying New York State tax on the wages you earned here. You normally also file a resident return in your home state, which may give you a credit for the New York tax.

Can nurses deduct union dues in New York?

Not on the federal return, but yes on the New York return if you itemize there. New York lets you itemize even if you take the standard deduction federally, and union dues count with other job expenses above a 2% of income floor.

Is nurse overtime tax-free in 2026?

Partly. Federally you may deduct up to $12,500 of qualified overtime ($25,000 joint), which is only the premium half of overtime required by federal wage law, and the deduction shrinks once modified AGI passes $150,000 ($300,000 joint).

What does a nurse's tax return cost?

Personal returns start at $300. Returns with several states or side business income are quoted after we see your documents, and you get the price in writing first.
